Partners Behavioral Health Management offers comprehensive care management services for individuals eligible for Local Management Entity/Managed Care Organization (LME/MCO) support, funded by both State and Medicaid resources. Care Management, an essential administrative service recognized by the Division of Medical Assistance (DMA) and the NC Division of Mental Health/Developmental Disabilities and Substance Abuse Services (NCDMHDDSAS), ensures individuals receive coordinated and effective care.

Guidance for Completing the Referral Form:

When completing the Partners Care Management Referral Form, please adhere to the following guidelines to ensure efficient processing and to avoid delays:

  • Complete Demographic Form and Relevant Referral Page: Ensure both the Demographic Form and the referral page most pertinent to the individual’s needs are fully completed. Only these sections are required for submission.
  • Mandatory Fields: All fields within both the Demographic Form and the selected referral page are mandatory. If a field is not applicable, please enter “NA” to indicate this.
  • Referral Page Selection: Choose the referral page that accurately reflects the individual’s primary needs:
    • Page 3: State-funded individual with Mental Health, Substance Use, Intellectual Developmental Disability, or Outpatient Commitment.
    • Page 4: Medicaid Recipient Receiving Crisis or Inpatient Services.
    • Page 5: Medicaid Recipient with an Intellectual or Developmental Disability.
    • Page 6: Medicaid Recipient – Child Mental Health.
    • Page 7: Medicaid Recipient – Adult Mental Health.
    • Page 8: Medicaid Recipient – Child or Adult Substance Dependent.
    • Page 9: Medicaid Recipient – Child or Adult co-occurring conditions (any combination of MH/SA/IDD).

Submitting Supporting Clinical Documentation:

To facilitate a comprehensive review and expedite the referral process, providers are requested to upload all relevant clinical documents into the Member/Enrollee Alpha+ record. Pertinent documents include:

  • Comprehensive Clinical Assessments
  • Psychological Evaluations
  • Medication Lists
  • Person-Centered Plan/Individual Support Plan
  • Hospital Discharge Paperwork
  • School Documentation

Referral Processing Times:

  • Outpatient Referrals: For outpatient referrals, the Regional Care Management Supervisor/Manager will notify you within three business days regarding the individual’s qualification for Care Management services.
  • Psychiatric Inpatient Referrals: For individuals admitted for Behavioral Health Inpatient services, the assigned Care Coordinator will contact you within two business days.
