If you previously accessed information in your portal but are now unable to, you may need to re-establish your connection with Primary Care Partners. Here’s how:

- If you do not see Primary Care Partners listed, click on “Add Connection.”
- Enter the zip code 81501 to search for Primary Care Partners.

- Accept the Release of Information to authorize the connection.
After completing these steps, your connection will be verified. You will receive an email notification within 5 to 7 business days to finalize the account connection.

FollowMyHealth allows parents and legal guardians to manage the health records of children and dependent adults through proxy access. In accordance with HIPAA and privacy regulations, proxy access is categorized as follows:
- Minor Proxy Access: Parents or guardians can establish an account to access and manage a child’s health records until the child reaches the age of 12.
- Young Adult Proxy Access: For teenagers, parents/guardians can have limited access, primarily for secure messaging with the Primary Care Partners care team.
- Adult Caregiver Proxy Access: Individuals with Medical Power of Attorney can request access to manage the FollowMyHealth account of a dependent adult.
