To determine if you qualify for these valuable benefits, eligibility is based on having one or more of the following chronic conditions: cardiovascular disorder, diabetes, disabling mental health condition, lung disorders, or obesity. It’s important to note that while these conditions are key factors, having a chronic condition alone doesn’t automatically guarantee enrollment in these benefit programs.
