Aging Partners’ First Service desk serves as your initial point of contact for information, exploring options, and arranging services or referrals. We are here to support individuals aged 60 and over, as well as their families, throughout our eight-county service area. Our dedicated counselors specialize in connecting people with vital community resources, providing comprehensive information about a wide array of available services, and sharing knowledge of state and federal programs that may be beneficial.

Care Management and Social Work Services
Aging Partners provides professional Care Management and Social Work services, offering essential support in assessing needs and coordinating services for individuals aged 60 and over. These services are particularly crucial in supporting our more vulnerable and frail senior population.
Our care managers, comprising professional social workers and nurses, are expertly trained to evaluate the living situations of frail or vulnerable older adults. They connect clients with appropriate community services and resources to effectively address their care needs. To gain a comprehensive understanding of each client’s unique situation and required services, our care managers conduct in-home visits. Care planning begins with a thorough assessment of individual needs, abilities, circumstances, and existing support systems. Based on this assessment, a detailed home care plan is developed. With the client’s consent and active participation, Aging Partners Personal and Family Services coordinates the delivery of various essential in-home services. Each client’s needs and care plan are continuously monitored and regularly reviewed. Adjustments are made proactively to ensure ongoing safety and maintain a high quality of life at home over the long term.
Social work services offer a short-term, focused support relationship. Our social workers provide the same high-quality professional services as care management, but with a less intensive and more temporary approach. Social workers are invaluable in assisting with navigating challenging life transitions, managing recertification processes, clarifying complex applications, and effectively collaborating with other agencies on the client’s behalf.
